Preparation for Appointment

Please remember to list all items asked for in the following questionnaire. Preparing your bankruptcy paperwork properly takes a lot of time and full disclosure of pertinent information. If we work on this together, we can give you the fresh start the law intends for you to have.

In addition to the attorney fees negotiated, before filing your paperwork we must have the filing fee of $209 for a Chapter 7 and $194 for a Chapter 13. We prefer a cashier's check or money order. It will expedite the processing of your case.

1) Please fill out EVERY question on each page to the best of your ability. If you are given a choice of YES or NO on this form, please check the correct choice. We will help you if you have questions.

2) Please write clearly or type your answers. We are relying on having accurate information from you so please make sure it is legible.

3) If you need more room to answer a question, then turn over the page and list the information on the back together with the number of the question you are answering.

4) Whenever a question asks you to be prepared to give any details, gather all papers concerning the matter, including bills and collection letters, and bring them with you to your appointment along with this form.

5) Finally, please remember to bring any of the following items with you if it pertains to your case:

a) Most recent statements/ bills of all creditors (all persons to whom you owe money);
b) Deeds and mortgages on your house or other real estate;
c) Any insurance policies;
d) Any papers relating to past bankruptcies you have filed;
e) Copies of tax returns for past two years;
f)  A copy of your most recent pay stub;
g) Any other papers you have concerning any of your debts;
h) Legal papers, complaints, judgments, divorce decrees;
i)  Any lease or installment sale ( lease purchase or rent-to-own) agreements for housing (apartment, mobile home, house, duplex) or other property (cars, televisions, etc.) that you have signed and that are still in effect or not fully paid.

Please complete all questions. If you and your spouse are not living together, and there is no possibility that your spouse will file bankruptcy along with you, then you don't have to answer the questions about your spouse.
